There are no refunds on lab test kits.
Pre-Approval is required on all returns. Opened bottles of supplements may not be returned. No supplements will be accepted after 30 days on all regularly stocked items. 25% of the product purchase price may be charged as a restocking fee depending on the manufacturer.
Refrigerated or perishable items can not be returned.
No refunds will be issued for office visits,
services or phone consults.
Send email request to:
firstname.lastname@example.org and request a return authorization number (RMA).
Ship the product back with you name, contact information and copy of receipt. Mark the RMA number clearly on the outside of the package.
